INSTRUCTIONS
Download the Membership Application and fill it out. Please use black ink. Scan your completed and signed application and proofs, then email them to: NSDOAF@gmail.com.
Next, proceed to pay the one-time Membership Fee by adding the NSDOAF Membership Fee for a Regular Adult Membership to your shopping cart, in the PayPal box below. We will contact you if we have any questions regarding your application.
Online Membership fee: $300.00, plus a 3% + 0.50 online transaction fee. Total: $309.50
